COVID-19 VACCINE SURVEY

As part of the district’s health and safety plan, all families and staff are asked to screen symptoms at home prior to leaving for school.  The information below is based on guidance received from the Pennsylvania Department of Health.

Before leaving for school, review these symptoms:

Fever
Chills
Cough
Shortness of Breath
Difficulty Breathing
New Loss of Taste or Smell
Muscle Pain
Headache
Sore Throat
Nausea or Vomiting
Diarrhea
Fatigue
Congestion or Runny Nose
 

If the student is exhibiting one (1) or more symptoms, stay home.


Please advise the appropriate school nurse if the student is exhibiting symptoms, has tested positive for COVID-19, or has been exposed to someone with COVID-19 symptoms.
