FALL SPORTS PHYSICALS
By appointment ONLY

     Wednesday May 21 – High School Nurse’s Office
           (for students in grades 9, 10 &11 during the 2007-2008 school year)

     Friday May 30th – Middle School Nurse’s Office
            (for students in grades 6, 7 & 8 during the 2007-2008 school year)

    Wednesday June 25 – High School Nurse’s Office
           (for students in grades 9, 10 & 11 during the 2007-2008 school year)

      Friday June 27 – High School Nurse’s Office
            (for students in grades 6, 7 & 8 during the 2007-2008 school year)

Interested student MUST pick up Athletic Forms and schedule appointments through the School Nurse Office

PIAA Comprehensive Initial Pre-Participation Physical Evaluation
